48 Year old female brought in by her husband.

C/C:
* Depressed Mood
* Social Withdrawal
* Anhedonia
* Severe Anxiety
* Insomnia
* Passive thoughts of wanting to die or disappear

> 3 year history of depression

Family H/O: Strong family history of depression & Anxiety

Meds: 5 antidepressants including combinations over 3 years with little consistent response.
Present Medications: Sertraline 100mg daily; and Alprazolam 0.5-1 mg daily

Q- What will you do next?
(a) Add an atypical antipsychotic
(b) ECT (Electroconvulsive Therapy)
(c) TMS (trancranial Magnetic Stimulation)
(d) Continue the same treatment
