Quiz: Selective Serotonin Reuptake Inhibitors
Q: Which of the following statement regarding SSRI (Selective serotonin reuptake inhibitors) is FALSE?
(a) Fluoxetine has shown positive benefit in bulimic patients but failed to show any benefit in prevention of relapse in anorexic patients.
(b) Among SSRIs, only Fluoxetine and Sertraline have pharmacologically active metabolites.
(c) Among cytochrome P450 enzymes, 2D6 is most important in drug interactions with most SSRIs.

Answer:
(b) and (d) are wrong statements above.
Fluoxetine has been shown to have a positive effect on the binge-purge cycle in some bulimic patients. The largest controlled study to date of fluoxetine in the prevention of relapse in anorexic patients failed to show any benefit for fluoxetine in preventing relapse relative to placebo
Source: Walsh et al. JAMA 295(22):2605–2612, 2006.
Among SSRIs: Fluoxetine (norfluoxetine with half life of 7–14 days), Sertraline (N-desmethylsertraline with half life of 2–3 days) and Escitalopram (S-demethylcitalopram) have pharmacologically active metabolites.
Among cytochrome P450 enzymes, 2D6 is most important in drug interactions with most SSRIs.
Long acting formulations of SSRIs: Fluoxetine (90mg once weekly), Paroxetine CR & FLuvoxamine CR.
Sertraline have more linear dose response curve, whereas others have relatively flat dose response curve
